Know every medicine you use

Keep one current list with the generic and brand names, strength, dose, timing and reason for each prescription, over-the-counter medicine, vitamin and supplement. Include allergies and previous serious reactions. Bring the list to appointments and hospital visits.

Check before taking

Read the label and active ingredient each time. Similar packaging and combination cold or pain products can lead to accidental duplicate doses. Use the supplied measuring device for liquid medicine, not a kitchen spoon. Never share prescription medicine.

Ask a pharmacist or prescriber before crushing tablets, mixing medicine with food, drinking alcohol, or using supplements. Do not stop long-term medicine suddenly unless advised.

Store and dispose safely

Keep medicines in original labelled containers, away from heat, moisture and children. Follow local pharmacy advice for disposal rather than keeping unneeded medicine indefinitely. Review the list after every hospital discharge or prescription change.
