A hot joint has several possible causes

Gout can cause rapid, severe pain, swelling, redness and tenderness when urate crystals inflame a joint. Infection and other inflammatory conditions can look similar, especially during a first episode. Examination and sometimes joint fluid or blood testing may be needed.

Treat attacks and long-term risk separately

Medicines used during a flare differ from long-term urate-lowering treatment. Choice depends on kidney function, other conditions and current medicines. Do not start, stop or double treatment without advice. Hydration and a sustainable balanced diet can support care, but diet alone may not adequately manage recurrent gout.

Persistent morning stiffness, several swollen joints, psoriasis, rashes, eye inflammation or unexplained fatigue may suggest another rheumatic condition and should be discussed.

Track a pattern

Note which joints are affected, how quickly symptoms began, fever, recent illness and current medicines. If gout is diagnosed, ask about a target urate level, monitoring and what to do during future flares.

At a glance

Key takeaways

  • A first hot, swollen joint needs assessment because infection can resemble gout.
  • Long-term urate-lowering medicine is different from short-term flare treatment.
  • Report symptoms involving several joints or other body systems.

When to seek care

  • Seek urgent same-day care for a hot swollen joint with fever, chills, marked illness or inability to use the limb.
  • Arrange review for recurrent attacks, persistent swelling, prolonged morning stiffness or medicine side effects.
