After a twisting injury
An anterior cruciate ligament injury may follow a sudden pivot, awkward landing or direct impact. A pop, rapid swelling or giving way can occur, but these features do not confirm the diagnosis. Other ligaments, cartilage or bone may also be injured.
Protect the knee and arrange assessment
Stop the activity, protect the knee, use wrapped ice briefly for comfort, elevate it and avoid repeatedly testing instability. Seek assessment if weight-bearing is difficult, swelling is substantial or the knee locks or gives way. Imaging may be used when it will clarify associated injury or guide management.
Not every ACL injury requires the same treatment. Rehabilitation is central, while surgery may be considered according to instability, associated damage, sport or work demands, age, health and informed preference.
Return by function, not the calendar alone
Rehabilitation usually progresses movement, strength, balance and task-specific control. Returning too early can increase reinjury risk. Follow objective milestones set by the orthopaedic and physiotherapy team.
